New to Tampa Bay DBT Counseling Center?
If you are visiting the office for the first time, you are encouraged to complete these forms ahead of time. These forms will help us gather important information about you and your health history. Please bring the completed forms with you to the first appointment.
Please note that the fee for the initial assessment is $150. Tampa Bay DBT does not participate with any insurance companies, but will provide you with a receipt for services that you can submit independently. A sliding scale is available and will be discussed during the initial assessment. All fees are due at the time of service. Tampa Bay DBT accepts cash, checks and credit cards (Visa, Master Card, American Express).
